Provides case coordination and delivers community rehabilitation services. The role involves conducting home visits, obtaining psychosocial histories, conducting functional skills assessments, and establishing liaison with psychiatric care providers. The position ensures client access to psychiatric, substance use, medical services, and entitlement benefits, while monitoring client medication and administering health screening tools. A key responsibility is developing and implementing individualized rehabilitation plans (Service/Support Plans) in consultation with the treatment team and client. The role also includes delivering various community rehabilitation services such as Assertiveness/Self Advocacy Training, Community Integration Services, Daily Living Skills Training, Health Services, Medication Management Training, Parenting Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Services, and Symptom Management. Additionally, the position offers housing support services in areas like education/employment support, behavioral health, harm reduction, entitlement assistance, healthcare, emergency services, Health Home Care Coordination, and transportation. The Case Manager documents client progress, updates plans, trains clients in safety procedures, prepares residents for independent living, facilitates crisis intervention, and staffs the Emergency Phone on a rotating basis. Maintaining client charts, submitting notes and reports, overseeing apartment maintenance, and attending in-service training are also required. The role may involve providing coverage for other staff, serving as a "Qualified Mental Health Staff Person," and performing other duties as assigned by the supervisor.
