About The Position
Deaconess Home Health is seeking a skilled and compassionate Speech Language Pathologist to provide home-based care in the Evansville, Indiana service area, which includes Posey, Vanderburgh, Warrick, Spencer, and Gibson counties. The therapist assesses, diagnoses, and treats patients with speech, language, communication, cognitive-communication, and swallowing disorders. The role involves developing individualized treatment plans and collaborating with families and other healthcare professionals to support patient outcomes. NestMed is used for AI documentation. The position has no after-hours on-call requirements.
Requirements
- Master’s degree in Speech-Language Pathology from an accredited program.
- Active registered SLP license in the state of practice and ASHA CCC-SLP certification.
- Obtain and maintain current CPR/BLS certification.
- One year of Speech Therapy experience in a clinical setting.
- Current driver’s license.
- Comply with federal, state, and local laws and regulations.
Responsibilities
- Provide therapy to patients in their homes to improve speech, language, cognitive-communication, and swallowing skills.
- Customize therapy strategies to meet individual patient goals and objectives.
- Maintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.
- Train and support family members in therapeutic exercises and strategies to use at home.
- Assess, diagnose, and treat speech, language, communication, and swallowing disorders.
- Collaborate with families and other healthcare professionals to achieve optimal patient outcomes.
