Speech Language Pathologist (SIGN ON BONUS) - Chesterfield, MI
About The Position
FOX Rehabilitation is expanding in Chesterfield, MI, and is seeking dedicated, licensed Speech Language Pathologists (SLPs) to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. Utilizing the Geriatric House Calls™ model, you will help patients enhance their mobility, independence, and overall quality of life. This role is situated within one of our Senior Living Community partners, Commonwealth New Baltimore. The position is ideal for SLPs who value autonomy, flexibility, and making a significant impact on patient outcomes, with the freedom to manage their own schedules and foster strong patient relationships. The role may also incorporate telehealth and virtual care options, offering increased flexibility, improved care continuity, and the ability to serve a larger patient base. Requirements
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
Responsibilities
- Provide one-on-one Speech Language Pathology to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
