About The Position

TMG is seeking an IRIS Consultant (IC) with a social services or human services background to support people enrolled in Wisconsin’s IRIS Program, a Medicaid long-term care option for older adults and people with disabilities. The position is remote and home-based, with regularly scheduled visits to participants in their homes and communities. The role requires building trusting relationships, helping participants navigate program requirements and budgets, documenting activities and changes in the IRIS data system, and collaborating with agency staff and other parties involved in implementing participants’ plans. The position includes in-person participant visits and monthly in-person team meetings, along with possible weekly phone check-ins with the supervisor.
