About The Position

The IRIS Consultant (IC) partners remotely with people in the Wisconsin IRIS Program and TMG IRIS Consultant Agency to help them navigate the program and achieve their long-term care goals. The position is home-based and includes regularly scheduled visits with participants in their homes and communities. ICs help participants identify goals and develop customized IRIS plans related to areas such as employment, housing, health, safety, community membership, transportation, and relationships. They connect participants with community resources and support them in directing their own services and budgets. The IC provides program orientation, helps participants understand their rights and responsibilities, and supports planning and implementation in accordance with IRIS program requirements and participant budgets. The role includes documenting visits and plan activities in the WISITs data system, researching community resources, explaining budget reports, coordinating with Fiscal Employer Agencies, and supporting matters such as provider billing, worker payments, receipts, support brokers, and risk mitigation. The IC maintains confidential records, communicates program changes and compliance requirements, collaborates with agency staff, and attends team meetings and supervisor check-ins.
