About The Position

The IRIS Consultant (IC) partners with people enrolled in Wisconsin’s IRIS Program, a Medicaid long-term care option for older adults and people with disabilities. The role builds trusting relationships and helps participants navigate the program, identify long-term care goals, connect with community resources, and develop customized plans related to employment, housing, health, safety, community membership, transportation, and relationships. This is a remote, home-based position with regularly scheduled visits to participants in their homes and communities. Consultants meet each participant in person at least four times per year, including one required annual home visit, and must be available upon participant request. The role includes orienting new participants; helping them understand their rights, responsibilities, and program requirements; supporting their outcomes within their IRIS budget; documenting activities and plan changes in WISITs; and assisting with budgets, provider billing, receipts, support brokers, and worker payments. Consultants also coordinate with Fiscal Employer Agencies, IRIS Consultant Agency staff, and other parties, maintain confidentiality and HIPAA compliance, and attend monthly in-person team meetings and possible weekly phone check-ins with a supervisor.
