About The Position

The IRIS Consultant (IC) partners with people enrolled in Wisconsin’s IRIS Program, a Medicaid long-term care option for older adults and people with disabilities. The position builds trusting relationships and helps participants navigate the program, identify long-term care goals, connect with community resources, and develop customized plans addressing areas such as employment, housing, health, safety, community membership, transportation, and relationships. The position also requires in-person monthly team meetings and may include weekly phone check-ins with the supervisor.
