About The Position

TMG is seeking an IRIS Consultant (IC) with a social services or human services background to support people enrolled in Wisconsin’s IRIS Program, a Medicaid long-term care option for older adults and people with disabilities. The role is remote and home-based, with regularly scheduled visits to participants in their homes and communities. ICs build relationships with participants, help them navigate the program, identify long-term care goals, and develop customized plans to pursue those goals. The work supports participants in areas including employment, housing, health, safety, community membership, transportation, and lasting relationships. The IC connects participants with community resources and natural supports, provides program orientation, and helps ensure plans meet participants’ personal outcomes while staying within their IRIS budgets and program requirements. The role also includes documentation, budget-related assistance, coordination with fiscal and provider agencies, and communication about program changes and compliance. ICs work collaboratively with colleagues and attend monthly in-person team meetings, with weekly phone check-ins potentially occurring.
