About The Position
We are seeking a dedicated Social Worker to join the Clinical Resource Management team. The role provides social work services, including discharge planning, care coordination, and care management for patients in inpatient settings. The position is listed as 40 guaranteed hours per week, with 10-hour shifts Monday through Friday, 8:30 AM to 5:00 PM, and a rotating weekend requirement. The dress code is business casual. Orientation is provided onsite and through online platforms. The role requires a valid driver's license for commuting purposes and adherence to the fingernail policy, which prohibits artificial nails in all patient care departments.
Requirements
- 2 years of relevant experience in social work.
- BLS Certification (AHA).
- NE CSW License.
- BSW (Bachelor of Social Work) for office roles.
- MSW (Master of Social Work) required for inpatient Emergency Department/NICU roles.
- Valid driver's license for commuting purposes.
- Adherence to the fingernail policy prohibiting artificial nails in all patient care departments.
Nice To Haves
- Experience in inpatient settings, with preference for prior Emergency Room experience.
Responsibilities
- Conduct discharge planning and ensure seamless transitions of care for patients.
- Coordinate care management services in collaboration with multidisciplinary teams.
- Assist patients in navigating social services and community resources to optimize care.
- Evaluate and assess patients’ social, emotional, and environmental needs.
- Maintain accurate and timely documentation in the electronic medical record (EMR) system, Epic.
