About The Position

Prepares patients and/or their families to transition from the hospital to the next appropriate care setting based on individual needs. Applies knowledge of medical conditions, insurance coverage, and the ways physical and psychosocial factors may affect patients after they leave the medical setting to plan and initiate individualized discharge plans. Provides clinical information to payers and/or post-acute providers to obtain authorizations for hospitalized patients.

Requirements

• Bachelor of Social Work from an accredited institution, unless a Master of Social Work is required for licensing.
• Licensed Medical Social Worker in Alabama (e.g., LSW, LCSW, ACSW, or equivalent), or obtain Alabama licensure within six months of employment.

Nice To Haves

• Master of Social Work from an accredited institution.
• Three or more years of acute care hospital experience.
