About The Position

The Social Worker in Case Management coordinates the interdisciplinary plan of care for patients from admission through discharge. The role develops care plans across the continuum while integrating patient and family preferences and values. It monitors care through assessments, evaluations, and patient records; advocates for needed resources and the removal of barriers; and maintains communication with the supervisor and other health providers to support effective implementation of the health plan. The Social Worker also serves as a resource for medical record documentation, the appropriateness of services in relation to diagnoses, and post-discharge treatment options. The position is based at UAB ST Vincent's Birmingham and is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m., with holiday rotations and a Saturday rotation every eight weeks. A bachelor's degree in Social Work and current Alabama licensure as an LBSW or LMSW are required.

Requirements

Bachelor's degree in Social Work.
Currently licensed as a Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners.
