Social Worker - Case Management - PRN Days
About The Position
The Social Worker in Case Management coordinates the interdisciplinary plan of care from patient admission through discharge. The role develops care plans across the continuum while incorporating patient and family preferences and values. It assesses, evaluates, recommends, and provides support for legal, neglect, substance abuse, crisis, psychosocial, and adjustment needs; advocates for resources and helps remove barriers; and maintains ongoing communication with the supervisor and care transition team to implement and reevaluate the health plan. The social worker also supports appropriate medical record documentation, complex level-of-care recommendations, and services related to diagnoses, treatment options, and postdischarge care. This PRN position is based at UAB St. Vincent’s Birmingham and may include weekend and holiday shifts as needed. Requirements
- Bachelor’s degree in Social Work.
- Current Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) license from the Alabama State Board of Social Work Examiners.
