Social Worker - Case Management - Days
About The Position
The Social Worker works in the Case Management department at UAB ST Vincent's Birmingham. This role coordinates the interdisciplinary plan of care from patient admission through discharge, develops care plans across the continuum that incorporate patient and family preferences and values, and monitors care through assessments, evaluations, and patient records. The Social Worker advocates for resources and removal of barriers, maintains communication with the supervisor and other health providers to support effective implementation of the health plan, and provides guidance on medical record documentation, service appropriateness in relation to diagnoses, and post-discharge treatment options. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with holiday rotations and a Saturday rotation every eight weeks. Benefits are offered for eligible positions only.
Requirements
- Currently hold an Alabama State Board of Social Work Examiners license as a Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW).
- Bachelor's degree in Social Work.
Responsibilities
