Social Worker - Case Management - PRN Days
About The Position
The Social Worker in Case Management coordinates the overall interdisciplinary plan of care for patients from admission through discharge. The role develops care plans across the continuum that incorporate patient and family preferences and values. The Social Worker assesses, evaluates, recommends, and treats legal, neglect, substance abuse, crisis, psychosocial, and adjustment needs; advocates for resources and removal of barriers; and maintains ongoing communication with the supervisor and care transition team to support effective implementation and reevaluation of health plans. The role also provides guidance on medical record documentation, complex level-of-care recommendations, and services related to diagnoses and treatment options for post-discharge care. This is a PRN position with day scheduling, including weekend and holiday shifts as needed, at UAB St. Vincent’s Birmingham. Requirements
- Bachelor’s degree in Social Work.
- Current Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) credential from the Alabama State Board of Social Work Examiners.
