About The Position
Community Care is hiring a full-time Social Worker for its Program of All Inclusive Care (PACE), based at its Racine, Wisconsin location. The role provides direct social work case management services to members and supports collaborative care planning. It offers weekday hours and is an on-site position. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible in their communities. It coordinates supportive services for more than 13,000 Wisconsin residents across the 23 counties it serves, working with members and community partners to address health and social needs.
Requirements
- Master’s degree in Social Work.
- Current Wisconsin Social Work licensure.
- At least 1 year of experience providing care or services for a frail or elderly population.
- For occasional local travel, a valid driver’s license and proof of required insurance coverage.
Nice To Haves
- 1 to 2 years of case management experience with elderly and/or developmentally or physically disabled populations.
Responsibilities
- Provide direct social work case management services in Community Care’s Program of All Inclusive Care (PACE).
- Maintain documentation standards and member-related paperwork.
- Facilitate team meetings.
- Occasionally travel locally using a personal vehicle.
