About The Position
Community Care is hiring a full-time Program Assistant/CNA for its Racine adult day center at 5200 Washington Ave. The role supports elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. Under the supervision of the assigned Day Center Supervisor or designee, the Program Assistant/CNA provides care and services addressing attendees’ personal and recreational needs, support, and health status. The role has a strong recreation focus and includes leading activities. The position provides personal and restorative care according to members’ care plans, keeps the team informed about members’ physical, emotional, or behavioral changes, and recommends care plan changes. It also leads and supervises group and individual activities, projects, and special events under the direction of Recreation Specialists or a designated supervisor, assists with daily cleanup of recreation areas, and delivers written restorative programs, including therapeutic exercise groups, with related documentation.
Requirements
- At least one year of experience providing care or services for frail elderly people and individuals with physical or developmental disabilities.
- Wisconsin Certified Nursing Assistant license in good standing on the registry.
- Active CPR and AED license.
- High school diploma; a GED may be substituted.

Responsibilities
