About The Position

Pathways Hospice & Transition Home Healthcare is hiring a Social Worker to support its hospice mission and Core Values. The role focuses on improving or maintaining patients’ ability to address everyday physical, mental, emotional, spiritual, and psychosocial needs. The position is PRN and provides coverage in the North and Northwest service areas.

Requirements

• Must be a Licensed Social Worker in the State of Texas.
• Must possess a bachelor's or master's degree in Social Work.
• At least two years of social work experience in a supervisory capacity in hospice or another related medical home health field.
• Outstanding organizational skills and the ability to deal tactfully with associates, patients, family members, visitors, government agencies and personnel, and the general public.
• Proficiency with computer systems, including Word, Excel, and an Electronic Health Record system.
• Must be in good standing with the Federal and State Government.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the hospice patient social services program.
• Interview patients and families during admission to complete a social history and assessment, identify problem areas, and develop rapport.
• Explain and obtain advance directives, including Directive to Physician and Medical Power of Attorney documents.
• Work with the Nursing department to ensure accurate Do Not Resuscitate orders are obtained for patients.
• Develop and implement policies and procedures for identifying patients’ medically related social and emotional needs.
• With qualified supervision or the appropriate license, apply social work theory, knowledge, methods, ethics, and professional use of self to restore or enhance patients’ social, psychosocial, or biopsychosocial functioning.
