About The Position

The Social Worker works in Social Services at Ascension Saint Agnes Hospital in Baltimore, Maryland. The role assesses patients’ social, emotional, and environmental needs and develops compassionate, personalized intervention plans. It supports post-discharge transitions through concurrent reviews, discharge planning, home care, and continuing social services. The Social Worker connects patients and families with community resources and coordinates care strategies with multidisciplinary teams, external organizations, and loved ones.

The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with no weekends or holidays. After six months of employment, the position includes one remote day.

Requirements

• A bachelor's degree in Social Work is required at the date of hire.
• A Social Worker credential from the Maryland Board of Social Work Examiners is required before the hire date or job transfer date; any level is accepted.

Nice To Haves

• A driver's license credentialed by the Maryland Department of Motor Vehicles is preferred.

Responsibilities

• Assess patients’ social, emotional, and environmental needs and develop personalized intervention plans.
• Support post-discharge transitions through concurrent reviews, discharge planning, home care, and continuing social services.
• Connect patients and families with resources by collaborating with community agencies, healthcare partners, and support networks.
• Coordinate care strategies with clinical managers, external organizations, and patients’ loved ones to support complete treatment compliance.
