About The Position

The Social Worker II is responsible for case coordination with insurance and medical staff within the Skilled Nursing Facility Unit. This role provides a full range of social work services, including psychosocial assessment, treatment planning, and clinical/therapeutic intervention to patients, their families, and caregivers. The position operates within an interdisciplinary environment and supports the continuum of care, including post-discharge follow-up for both the Skilled Nursing Facility and inpatient units of the General Acute Care Hospital. The Social Worker II may receive assistance from the clinical nurse manager and program leadership. Key activities include attending interdisciplinary meetings and family conferences to provide input and feedback on patient progress and needs, and participating in continuum of care initiatives. The role requires adherence to Desert Regional Medical Center's Mission, Vision, and Values.

Requirements

  • Master’s degree in Social Work (MSW) from an accredited college or university (or equivalent).
  • Current American Heart Association BLS Certification (maintain current at all times).
  • Effective verbal and written skills to communicate with people on all levels.
  • Ability to maintain positive working relationships with patients, families, physicians, and co-workers.
  • Possess active listening skills.
  • Ability to independently manage multiple priorities.
  • Excellent computer skills (i.e. eRehab Data, Medilinks, Cerner, Allscripts software).
  • Demonstrate teamwork in a pleasant manner.
  • Ability to write legibly.

Responsibilities

  • Participate in an interdisciplinary environment.
  • Provide follow-up post-discharge for both the Skilled Nursing Facility and inpatient units of the General Acute Care Hospital.
  • Attend interdisciplinary meetings and family conferences to receive input and provide feedback on the person served, progress, and needs.
  • Participate in and demonstrate accountability for continuum of care, including post-discharge follow-up.
