About The Position

The Social Worker is responsible for providing medically-related Social Service programs to meet the needs of residents. This includes determining the type of assistance required and assisting residents in obtaining benefits from various agencies and organizations such as Medicare, Medicaid, Social Security, Veterans Benefits, Psychiatric, Psychology, Dental, Vision, and others.

Requirements

  • Compassion for and desire to work with seniors!
  • Minimum Bachelor's Degree or higher in Social Work from a state-accredited program or bachelor's degree with one-year experience providing social services in a healthcare setting.
  • 1-year social work experience in the healthcare field.
  • Excellent interpersonal skills and working knowledge of community resources.
  • Excellent communication skills and knowledge of various computer programs, including Microsoft Office and web-based software systems.

Responsibilities

  • Assesses residents to identify psychosocial, financial, medical, environmental, community, and discharge needs. Documents assessment, clinical team meeting minutes, and community resources referrals.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Attends Interdisciplinary team (IDT) meetings, participates in the patient care planning process, and collaborates with the IDT to promote coordination of patient care.
  • Coordinates effective patient discharges to the community. Communicates and documents planned discharge needs.
  • Demonstrates commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
  • Accepts clinical assignments that are consistent with education and competence to meet the needs of the patients.
  • Encourages, listens, respects, and reassures residents and families in all aspects of social service.
  • Participates in ongoing training and in-services for improving knowledge and updating skills.
  • Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions that assist others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors.
  • Seeks out and performs other duties as assigned or needed.
