About The Position

The Social Worker MSW is responsible for coordinating patients’ discharge plans throughout their hospital admissions, beginning at admission and continuing through discharge follow-up. The role provides biopsychosocial assessments and updates to address patients’ changing discharge care needs and support timely transitions to the appropriate next level of care. In collaboration with the multidisciplinary team, the social worker develops plans of care that provide maximum benefit to patients and their families and participates in Care Coordination Rounds (CCRs) seven days a week.

Requirements

A master's degree in Social Work is required.
A current State of Wisconsin social worker license is required.
Interpersonal skills to obtain information from patients, provide counseling to patients and families, and interact effectively with them.
Analytical skills to assess patient needs, develop discharge plans, and develop solutions to concerns involving patients’ mental and physical well-being.

Nice To Haves

A minimum of two years of experience in an acute care setting is preferred.
Casework experience in medical social work is preferred.

Responsibilities
