About The Position

This position promotes patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources. The Social Worker manages, supports, and develops comprehensive transitional care plans for patients with complex and psychosocial needs in inpatient and Emergency Department (ED) environments. The role promotes quality of care and effective transition planning through collaboration with service team members, physicians, patients, external agencies, and families.

Requirements

• Master’s Degree in Social Work from an accredited social work program.
• One year of experience in an acute or post-acute setting.

Nice To Haves

• LMSW - Licensed Masters Social Work.
• Accredited Case Manager (ACM).
• CCM - Certified Case Manager.
