About The Position

The Social Worker is responsible for completing biopsychosocial assessments of patients and developing patient care plans for post-discharge services. Discharge planning may include referrals to home health, durable medical equipment (DME), dialysis, short-term rehabilitation, and nursing home placements. The Social Worker also serves as a resource for staff, patients, and families regarding community agencies, manages adoptions, and assists with special programs and education.

Requirements

• Master’s degree in Social Work.
• Two years of experience in hospital or community social work.
• Case management experience.
• Knowledge of medical and social work terminology.
• Ability to use standard office equipment, including computers, printers, fax machines, and CRT systems.

Responsibilities

• Complete biopsychosocial assessments of patients.
• Develop patient care plans for post-discharge services.
• Coordinate discharge planning and referrals to home health, DME, dialysis, short-term rehabilitation, and nursing home placements.
• Serve as a resource for staff, patients, and families regarding community agencies.
• Manage adoptions.
• Assist with special programs and education.
