About The Position

The Social Worker MSW Pool position in Care Management facilitates care for designated patients and their families during hospital admissions or outpatient visits. The role coordinates discharge planning from pre-admission through discharge follow-up, provides psychosocial assessments and updates addressing patients’ evolving needs, and establishes plans of care designed to provide maximum benefit to patients and families. Other duties may be assigned. This position is on-site at Froedtert Hospital in Milwaukee, Wisconsin.

Requirements

• Master’s degree in Social Work with casework in medical social work.
• Current State of Wisconsin social worker license required.
• Interpersonal skills to obtain information from patients, provide counseling to patients and families, and interact effectively with patients, families, and hospital personnel.
• Analytical skills to assess patient needs, develop discharge plans, and develop solutions to concerns regarding patients’ mental and physical well-being.

Nice To Haves

• A minimum of two years of experience in an acute care setting is preferred.
