An employee in this classification provides professional social work services to Social Services clients in the Community Alternatives Program (CAP) and other Family Services. This position provides case management services to disabled adults and their families. The social worker provides services to assist clients and their families in understanding their medical diagnoses and treatment plan and to adapt and cope with their problems. This worker completes an initial assessment to evaluate activities of daily living, instrumental activities of daily living, social environment, economic resources, and home environment. Worker also completes a cognitive, social, mental, and physical evaluation in sometimes complicated and/or questionable cases to fully and accurately assess client needs and determine the best possible treatment and intervention plan. A treatment plan is developed to meet the unmet needs of the client. The worker arranges for services and coordinates and interacts closely with other service providers to carry out the treatment plan. Position must keep records and completes ongoing documentation. Worker is responsible for establishing and developing interdisciplinary and multidisciplinary teams designed to address the problems of and meet the unmet needs of the client. Worker works closely with other disciplines to assure efficient and appropriate delivery of quality client services. This position is responsible for the Crisis Energy programs and some guardianship cases. This position also performs other duties as assigned.
