About The Position

The Community Health Worker in Social Services will work collaboratively with a care team of doctors, nurses, social workers, and other specialists to coordinate care for patients and families, addressing both medical and social needs. This role involves conducting face-to-face visits to evaluate care plan progress, communicating findings, and coordinating next steps with the care manager. The Community Health Worker will monitor the ongoing needs of patients and families to remove barriers, ensure efficiency, and improve access to care and resources. Responsibilities include scheduling appointments, completing required documentation linked to the care plan, providing updates to families on patient condition and progress, and referring families to appropriate resources. The role also involves consulting with care team members to address complex issues and supporting population outreach to identify at-risk individuals and close care gaps.

Requirements

  • High school diploma or equivalent
  • 2-5 years of work experience in a related job discipline
  • Have a reliable vehicle/valid driver's license.

Nice To Haves

  • Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required.

Responsibilities
