About The Position

Join a Team That Empowers Seniors & Adults with Disabilities in Wake County! We’re looking for a compassionate, detail-oriented professional to join our dynamic team as an Adult & Community Services Social Worker! The Wake County Social Services Senior & Adult Services Program offers a variety of home and community-based programs and services to seniors, adults with disabilities, and their families. Our program is made up of three collaborative teams: Adult & Community Services, Adult Guardianship, and Adult Protective Services. Together, we work toward one mission: promoting safety, health, and independence so individuals can thrive in their communities for as long as possible. Additionally, we strive to maintain the highest quality of life for those who reside in adult and family care homes in Wake County. The Senior & Adult Services Program provides: (1) placement services, (2) case management services for people receiving Special Assistance/In-Home funds, (3) monitoring of adult care homes and day care programs, and (4) protective services to disabled adults who are alleged to been abused/neglected/exploited and in need of protection and (5) Adult Guardianship services act as a surrogate decision maker for adults who have been adjudicated incompetent by the courts. In this role, you will: Monitor family care homes, adult care homes, and adult day care/day health programs Ensure compliance with state licensure and certification requirements Support our mission to safeguard the well-being and dignity of vulnerable adults Develop and implement trainings in the community on issues pertinent to elderly and disabled adults Investigate complaints received on family care homes, adult care homes, and adult day care/day health programs and issuing citations as appropriate based on the outcome of investigations Evaluate and provide consultation and technical assistance to adult care home licensee and administrators for initial licensure and compliance with licensure rules Frequently work in the capacity of liaison between consumers and residents of adult care homes If you're passionate about advocacy, community impact, and helping individuals maintain independence, we’d love to meet you!

Requirements

  • Bachelor's degree in Social Work or related field
  • Two years of job-related experience
  • Valid Driver's License and "safe" driving record
  • Equivalent education and experience are accepted
  • Excellent communication and organization skills
  • Knowledge of resources for elderly and disabled adults with mental illness, intellectual developmental disabilities and substance abuse
  • Ability to read and understand legal, medical and mental health documents
  • Ability to prioritize and respond to the needs of a large caseload and ensure that projects within areas of responsibility are completed in a timely manner
  • Ability to identify issues, obtain relevant information, relate and compare data from different sources, and identify alternative solutions
  • Ability to develop and maintain effective relationships with others in order to encourage and support communication and teamwork
  • Ability to monitor programs and/or activities and take corrective action when necessary
  • Ability to firmly adhere to codes of conduct and ethical principles and make decisions without regard to personal prejudices, biases, and experiences
  • Ability to provide detailed professional documentation

Nice To Haves

  • Master's degree in Human service related field
  • One (1) year knowledge of adult care homes, adult day care centers and their regulations
  • Nine (9) months experience with monitoring
  • Experience working with elderly, disabled and adults with mental illness, intellectual developmental disabilities and substance abuse in licensed adult care homes, adult day care facilities and in the community
  • Experience working in fast paced environment
