About The Position

Goodhue County Health & Human Services is seeking qualified applicants for the position of Social Worker within Social Services. Under general supervision, provides Adult Behavioral Health Outreach, Pre-Petition Screening/Civil Commitment Case Management, and Adult Protective Services. The role involves serving as a member of the Adult Social Services Screening Team, investigating allegations of maltreatment, assessing the need for a Guardian and/or Conservator, conducting Pre-Petition Screenings for Civil Commitment, connecting individuals with community resources, coordinating behavioral health services, providing time-limited stabilization and case management, and serving on the Social Services Intake team.

Requirements

A Bachelor’s degree from an accredited college/university with a major in Social Work, Psychology, Sociology or closely related field.
A valid driver’s license.

Responsibilities

Serve as a member of the Adult Social Services Screening Team, receiving and evaluating Adult Social Services requests for service.
Investigate allegations of maltreatment, including abuse, neglect, and exploitation of vulnerable adults, in accordance with applicable statutes and policies.
Assess the need for a Guardian and/or Conservator and participate in related court processes.
Conduct Pre-Petition Screenings for Civil Commitment and prepare required documentation for court proceedings.
Connect individuals with appropriate community resources and support them in accessing services to meet identified needs.
Coordinate behavioral health services in collaboration with internal and external providers and other systems.
Provide time-limited stabilization and case management services to ensure appropriate supports are in place.
Serve as a member of the Social Services Intake team by receiving referrals, drafting intake documentation, and routing information according to established procedures.
