About The Position

The Social Worker Care Manager at St. Rita's Medical Center provides appropriate interventions and discharge planning services to patients and families, facilitating smooth transitions across the continuum of care by accessing hospital, community, and governmental resources. The position identifies and prioritizes patients needing social services through a holistic assessment of biopsychosocial, functional, cultural, spiritual, and financial factors. The Care Manager plans with patients, caregivers, and the healthcare team to support effective, high-quality, cost-conscious outcomes, and monitors or revises plans as patient conditions change. The role maintains clear, concise, timely documentation, including post-hospital care plans, available resources, and patient or caregiver involvement and preferences. It follows standardized processes for advance care planning, length-of-stay management, and readmission prevention; supports denial prevention and medical necessity; and helps remove barriers to timely testing, treatment, and progression of care.
