About The Position

The Social Services Director identifies and provides for each resident’s social, emotional, and psychological needs and supports the continuing development of each resident’s full potential during their stay at the facility. The role also assists with planning for resident discharge and transition. The position is at Signature HealthCARE of South Louisville, a 100-bed facility providing skilled care, intermediate care, and inpatient and outpatient therapies.

Requirements

• Bachelor’s or master’s degree in Social Work or Psychology.
• At least 3 years of experience working in Social Services directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Complete required documentation accurately and in a timely manner to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in discharge planning and prepare residents for transition.
• Serve as a general source of information for residents and their families.
