About The Position

Avalon Health & Rehabilitation Center is hiring a Social Services Director to lead the facility’s social services program and support residents’ emotional, social, and psychosocial well-being. The role includes advocating for residents, supporting individualized care planning, and helping residents and families navigate adjustment, discharge planning, and transitions of care. The director connects residents and families with community resources and collaborates with interdisciplinary team members to promote positive outcomes and support regulatory compliance and accurate documentation.

This is a full-time position in Pasco, Washington, with a Monday–Friday schedule. The stated pay range is $33.00–$35.00 per hour, based on experience, qualifications, and skills. Requirements

• Two years of social work experience required.
• Knowledge of state and federal regulations.
• Strong communication, assessment, and documentation skills.
• Basic computer proficiency.
• Ability to pass employment screenings.

• Bachelor’s degree in Social Work preferred.
• Healthcare or skilled nursing experience preferred.

Responsibilities
