About The Position
Marin Post Acute is seeking an experienced and compassionate Social Services Director to join its leadership team in San Rafael, California. The role supports seniors in a skilled nursing community by advocating for residents, coordinating discharge planning, supporting emotional well-being, and helping residents and families navigate healthcare with dignity, respect, and support. The director oversees the Social Services Department and helps ensure resident-centered care and compliance with applicable state and federal regulations.
Requirements
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field.
- Strong knowledge of discharge planning, care coordination, and resident advocacy.
- Excellent interpersonal, communication, and organizational skills.
- Compassionate, dependable, and committed to providing exceptional service to seniors.
Nice To Haves
- Skilled Nursing Facility (SNF), rehabilitation, long-term care, or healthcare experience.
- Previous leadership or supervisory experience.
Responsibilities
- Direct and oversee all aspects of the Social Services Department.
- Develop, coordinate, and implement resident-centered social service programs.
- Assist residents and families with admissions, care planning, transitions, and discharge planning.
- Advocate for resident rights and ensure compliance with state and federal regulations.
- Collaborate with physicians, nursing staff, therapy teams, and community agencies to support positive resident outcomes.
- Provide counseling, emotional support, and crisis intervention when needed.
- Build strong relationships with residents, families, and community resources.
- Maintain accurate documentation and participate in interdisciplinary care team meetings.
