About The Position

The Social Services Director oversees the development, implementation, supervision, and ongoing evaluation of the Social Services Department. The department is designed to help residents attain or maintain their highest practicable well-being. The role includes identifying and arranging medically related social services, supporting comprehensive assessments and care plans, overseeing preadmission services, coordinating safe transitions back into the community, and maintaining departmental quality assurance procedures. The position works in an organization that emphasizes local leadership, resident and family needs, and exceptional care outcomes.

Requirements

A minimum of a bachelor’s degree in social work or another human services field, including sociology, gerontology, special education, rehabilitation counseling, or psychology.
One year of supervised social work experience in a health care setting, working directly with individuals.
A social worker license in the required state.

Responsibilities

Identify residents’ needs for medically related social services and ensure services are provided in accordance with state and federal regulations.
Complete or direct/delegate completion of the social services component of the comprehensive assessment.
Contribute or direct/delegate contribution of social services goals and approaches to residents’ care plans.
Direct the Social Services Department, including overseeing preadmission services for inbound residents.
Facilitate residents’ safe transitions back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
Oversee the establishment of departmental quality assurance procedures and modify them where appropriate.
