About The Position

The Social Services Director provides services to address residents’ social and emotional needs that affect their ability to achieve their highest level of function. The director participates in developing residents’ comprehensive care plans and develops policies and procedures for providing social services in compliance with federal, state, and local regulations. The position is at a skilled nursing facility serving veterans in Cleveland, Tennessee.

Responsibilities

• Develop and implement social service policies and procedures that meet residents’ needs and comply with federal, state, and local regulations.
• Develop one-to-one professional relationships with residents and families for counseling as needed.
• Counsel residents and families about feelings concerning death or dying and other emotional, mental, environmental, or physical limitations.
• Document psychosocial needs, interactions, and follow-up actions with residents and/or families according to facility policies and procedures.
• Assess, plan, and document residents’ discharge needs in accordance with facility discharge-planning policy and regulatory requirements.
• Document the social service component of each resident’s Comprehensive Care Plan in a timely manner.
• Refer residents to social, health, and community agencies, and accurately document referral results in residents’ records.
• Plan, schedule, and conduct room visits for residents unable to leave their rooms.
