About The Position

Responsible for coordinating and directing Social Services in accordance with federal, state and local regulations, established procedural guidelines, and as directed by the Administrator. The role applies professional social service practice and promotes residents’ rights, dignity, individuality, and well-being. It responds to residents’ behavioral and psychiatric needs through assessments, treatment recommendations, and referrals, while coordinating discharge planning, advance directives, grievance resolution, family council meetings, and ancillary appointments.

Requirements

For buildings with more than 120 beds, a bachelor’s degree in Social Work or a Human Services field such as Sociology, Gerontology, Special Education, Rehabilitation Counseling, or Psychology.
For buildings with 120 beds or fewer, a bachelor’s degree is required.
At least one year of supervised social work experience in a healthcare or long-term-care setting, working directly with patients.
Previous supervisory experience.

Nice To Haves

Master of Social Work (MSW) preferred.

Responsibilities

Apply standards of professional Social Service work practice.
Demonstrate knowledge of and respect for each resident’s rights, dignity, and individuality in all interactions.
Protect, promote, and advocate for residents’ rights.
Respond to resident behavioral and psychiatric issues by completing behavioral and psychosocial assessments, providing treatment recommendations, and making referrals to appropriate mental and behavioral health providers.
Coordinate discharge plans with residents and families.
Assist patients and families with advance directives in accordance with federal regulations.
Resolve and track grievances.
Complete psychosocial assessments of residents.
Coordinate family council meetings.
Coordinate ancillary appointments as necessary.
