About The Position

Ohana Pacific Health is seeking a caring and compassionate Social Services Assistant to work with the interdisciplinary team. The role identifies and assesses guests’ psychosocial, emotional, environmental, spiritual, and support needs and helps develop a plan of care. Nice To Haves

• Bachelor’s degree in Social Work.
• Membership in the National Association of Social Workers and the Academy of Certified Social Work.
• Hawaii State Social Work license or eligibility for licensure.
• At least one year of experience providing social services or working in long-term care.

Responsibilities

• Assist with counseling and crisis intervention to support effective coping and problem-solving related to illness or disability, death, and emotional or relationship problems.
• Make daily rounds to all units and review communication logs, social service logs, and loss logs.
• Complete Social Services processes, including initial assessments, state and federal timed assessments, and care plans.
• Identify guests who need a power of attorney, surrogate, or guardianship, and coordinate the process to ensure each guest has a representative upon admission or within one month of admission.
