About The Position

A Social Service Coordinator is needed for a Senior Citizen Property. Essential Role and Responsibilities: Coordinate social services for residents that support positive development in education, health, and financial stability, that lead to self-sufficient individuals and families; Establish and maintain relationships with local service providers, schools, civic organizations, funding agencies, business partners and other community based organizations; Manage site program budget, review monthly reports, and ensure accuracy in reporting; Work with Company to build annual budget and determine priority areas for funding; Write and submit grant proposals to local funding organizations, as agreed upon by Company Meet program enrollment and attendance goals (daily and average) as agreed upon by SSC and Company Collect, manage, report and analyze data, and ensure accuracy and consistency with established data collection standards Analyze effectiveness of programs using collected data and determine site-specific program needs in addition to regular program offerings Conduct pre- and post-assessments as needed for internal reporting (directed by Company) and external reporting (directed by contractual agreements) Manage contracts for third parties providing programs on site under SSC's purview Provide monthly reports to Company Participate in program, staff and resident meetings as needed Attend required training courses annually Other duties as assigned

Requirements

  • A Bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable; a college degree is fully acceptable. However, individuals without a degree, but with appropriate work experience may be hired.
  • Training in the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse by the elderly, and mental health issues.
  • At least 3 years of social service experience, preferably in the homeless world
  • Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled served.
