About The Position

Requirements
- Valid LPN license in the state of Michigan.
- Experience working in skilled nursing or home health care settings.
- Proficiency with Kinnser software or similar home health care electronic documentation systems.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Reliable transportation to travel to patient homes within Oakland, Michigan.
Responsibilities
- Provide skilled nursing care to patients in their homes, following physician orders and care plans.
- Use Kinnser software to document patient care, observations, and clinical data accurately and promptly.
- Monitor patient health status and promptly report changes to the healthcare team.
- Administer medications and treatments as prescribed.
- Educate patients and their families on disease management, medication administration, and health maintenance.
- Ensure compliance with regulatory standards and company policies.
