Requirements

  • Bachelor’s degree from accredited college or university or equivalent experience required.
  • Minimum of 7 years experience in a healthcare setting.
  • Minimum of 2 years experience in managing employees.
  • Experience managing multidisciplinary care teams.
  • Experience working with homeless, criminal justice, or other underserved populations.
  • Understanding of social determinants of health and impact on patient well-being.
  • Culturally competent and able to work with a diverse population.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
  • Proficient in working with various electronic health record systems while maintaining the integrity and accuracy of the data.
  • Excellent verbal, written, and interpersonal skills, and strong problem-solving skills.
  • Excellent organizational skills and ability to multitask and juggle multiple priorities.
  • Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations.

Responsibilities

  • Directing, organizing, and managing Serene Health’s Community Support (CS) Program.
  • Managing health plans for referrals and authorizations.
  • Overseeing department assessments.
  • Ensuring housing deposit processes are completed accurately and with proper documentation.
  • Overseeing department at various sites throughout the state.
  • Developing program goals and objectives, established staffing patterns, and organizing the staff work.
  • Leading a multidisciplinary team in providing a seamless continuum of care and coordination for high-need, high-risk populations.
  • Acting as liaison with other CS departments and programs to create and maintain systems of care coordination.
  • Acting as liaison with external referral resources and programs to create and maintain systems of care coordination.
  • Interacting professionally with all Health Plan partners.
  • Designing and implementing systems of care coordination to enhance outcomes for high-risk patients.
  • Helping to create and provide oversight for community support activities.
  • Working with community support and healthcare teams to ensure a Health Action Plan (HAP) is created for all CS patients and other patients requiring care.
  • Ensuring that goals of individual patients’ HAP are being met.
  • Overseeing the development, maintenance, implementation, and training of protocols, policies, and procedures for all functions related to the community support.
  • Planning and conducting CS staff meetings on a regular basis.
  • Keeping abreast of CS rules and regulations to ensure ongoing program compliance.
  • Overseeing accurate documentation in Serene Health’s EHR system to ensure that program requirements are met, high-quality care is provided, and timely payment is received.
  • Extracting and reporting on various data sets from various systems.
  • Gathering productivity and quality data from CS System and other sources and using it to provide meaningful management information to appropriate staff.
  • Performing other duties as assigned.
