Shared Living Provider - Sturbridge

About The Position

Venture is seeking compassionate and committed shared living providers in Central Massachusetts to share their home and life with a person with a disability. The provider supports the person with daily activities and health and wellness, promotes and models a safe and meaningful lifestyle, and encourages participation in the community. Providers receive training and certifications, guidance and ongoing support from a case manager, clinical and nursing supports, 24/7 emergency on-call assistance, and a generous tax-exempt stipend. The provider is responsible for the complete and total care of the individual supported. The contract includes an allotted amount of respite care for the provider’s personal time, such as appointments, vacations, nights out, or other time away.

Requirements

Must be 21 years or older.
Must have an available bedroom that can be individualized as the person desires.
Must have a valid driver’s license.
Must own a reliable and insured vehicle.
Must pass a background and fingerprinting check.
Must pass the home qualification process.

Nice To Haves

Prior experience in human services is helpful but not necessary.

Responsibilities

Support the individual with daily activities and activities of daily living (ADLs).
Assist with the individual’s health and wellness.
Promote and model a safe and meaningful lifestyle.
Encourage the individual’s participation in the community.
Provide transportation to community activities.
Take the individual to all appointments.
Complete monthly documentation.
Administer medication.
Assist with money management.
Provide complete and total care of the individual supported.
