Position Summary and Scope of Service The Service Facilitator will take an active role in promoting service and developing creative strategies to work with families and systems. The Service Facilitator promotes a positive, nurturing environment for the family and works toward strengthening support networks, enhancing knowledge, competencies, skills, and family stability using a family-centered approach within an ecological model of practice. The Service Facilitator is responsible for evaluating the effectiveness and efficiency of direct interventions to ensure positive, observable, and beneficial outcomes to families. The Service Facilitator will provide direct clinical and consultative services to clients, families or family supports and is familiar with and promotes a “customer service focus” in the provision of services to clients, and other professionals. The Service Facilitator provides services with the Mission and Values of St.Amant, policies and procedures of Clinical Services, external legislation and regulatory bodies and all relevant professional standards of practice. The Service Facilitator promotes processes that contribute to the areas of self-determination and the development of skills and competencies of individuals and care providers. These areas pertain to the entirety of the intervention including assessments, treatment, and implementation of service. The Service Facilitator is responsible for evaluating effectiveness and efficiency of interventions to ensure outcomes that are positive and beneficial to clients. The Service Facilitator is familiar with social policy, issues, trends, best practices in the field, needs, gaps or opportunities that may improve the lives of persons with disabilities, or which promote social justice, equity or accessibility. The Service Facilitator is familiar with and adheres to all policies related to risk management and those of workplace activity, promotes and follows all safety, health and respectful workplace practices. The Service Facilitator is involved in a variety of services and has responsibilities including the provision of direct and indirect services to individuals and families, facilitating groups, participating in research or educational activities, facilitating access to services and maintaining statistics. The Service Facilitator is involved in St.Amant activities that are related to optimizing enhancement of quality of life of individuals, families and the community.
