About The Position
Moms In Motion is seeking a compassionate, organized Service Facilitator (SF) Case Manager to support families navigating Virginia Medicaid Waiver programs, including CCC+, CL & FIS, and EPSDT. The role helps clients live safe, healthy, and independent lives through in-home visits, care planning, assessments, and assistance with service-related challenges. The position involves daily local travel to clients’ homes in Virginia, along with remote work using a laptop or tablet. A degree is not required. Anticipated annual earnings generally range from $40,000 to $75,000 based on completing a minimum of 125 visits per month; actual earnings vary according to the number and type of visits completed.
Requirements
- At least 2 years of experience supporting individuals with disabilities or older adults.
- A valid driver’s license and reliable vehicle; transporting clients is not required.
- Technology skills for email, documents, spreadsheets, portal navigation, and video calls.
- Strong written and spoken communication skills.
- Ability to pass a background check.
- Provide 2 professional references.
- Must live in Virginia.

Responsibilities
