About The Position

The Service Coordinator is responsible for supporting resident stability and independence through structured service coordination, program implementation, and resource linkage. In accordance with HUD Notice 2023-02, Section IV, this position conducts required resident assessments, implements established service programming, and ensures accurate documentation, reporting, and outcome evaluation in alignment with HUD standards for success. This is a contracted 1099 position. Position requires a 40-hour work week on-site.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field required
  • Minimum of 3+ years of experience in social services, case management, or resident services
  • Experience using case management systems; Casebook preferred
  • Strong organizational, documentation, and communication skills
  • Ability to maintain confidentiality and exercise sound professional judgment

Nice To Haves

  • Experience with HUD-assisted housing programs preferred
  • Experience working with senior populations preferred

Responsibilities

  • Conduct resident assessments at move-in and annually thereafter to identify service needs and support planning.
  • Maintain accurate, timely, and confidential resident case files.
  • Provide referrals and connect residents to appropriate community-based services and supports.
  • Monitor resident engagement and follow up to support housing stability and well-being.
  • Implement and facilitate established monthly resident programs and classes in accordance with company-designed curriculum.
  • Support resident participation and engagement in assigned programming.
  • Coordinate logistics including scheduling, communication, and space preparation.
  • Collaborate with management and approved partners to ensure consistent service delivery.
  • Maintain and manage resident toolkits to ensure materials are current and accessible.
  • Complete an annual toolkit review and refresh in alignment with program updates and resident needs.
  • Distribute and educate residents on available toolkit resources.
  • Document all services, case notes, and resident interactions in Casebook case management software.
  • Ensure compliance with HUD requirements, funding standards, and internal documentation protocols.
  • Prepare and submit annual performance and outcomes reporting in compliance with HUD Notice 2023-02 standards for success.
  • Maintain audit-ready records and support monitoring or compliance reviews as required.
  • Serve as a liaison between residents, property management, and community service providers.
  • Support a positive, stable, and resource-connected residential environment.
  • Communicate effectively regarding available services and program opportunities.
