About The Position

SPM, LLC is seeking a passionate and dedicated Service Coordinator for New Pilgrim Towers in Birmingham, AL. This role is crucial in linking residents with supportive services to promote independence, enhance quality of life, and ensure safe housing. The Service Coordinator will conduct resident assessments, develop service plans, coordinate educational and wellness programs, provide referrals to community resources, maintain documentation, and collaborate with property management and local service providers, all while ensuring compliance with HUD regulations and company policies. Requirements

  • Bachelor’s degree in Social Work, Gerontology, Psychology, or related field (Required)
  • Knowledge of local services, state and federal programs, and senior resources.
  • Understanding of Medicare, Medicaid, Social Security and other public benefit programs for seniors.
  • Strong case management and problem-solving skills with a team-oriented attitude.
  • Excellent communication and relationship-building skills
  • Organized, resourceful, and ready to take initiative

Responsibilities

  • Empower residents to live independently by connecting them with essential services, community resources, healthcare providers, and social service agencies.
  • Build and maintain a robust network of local agencies, providers, and wellness resources.
  • Educate and guide residents and families through Federal, State, and local programs tailored to senior adults.
  • Assist residents in accessing benefits such as Medicare, Medicaid, SNAP, SSI, transportation, and other assistance programs.
  • Coordinate wellness and educational programs that promote healthy aging and independent living.
  • Conduct regular follow-up with residents to ensure services remain appropriate and effective.
  • Maintain accurate resident files and documentation in compliance with HUD and company requirements.
  • Complete assessments, referrals, case notes, and reports in a timely manner.
  • Respond to resident concerns and assist with problem-solving.
  • Build relationships with local service providers to expand available resources for residents.
  • Collaborate with the management team to identify residents in need and provide proactive assistance.
  • Advocate and negotiate on behalf of residents for cost-effective and adequate services.
  • Create and maintain an up-to-date directory of community resources for residents and staff.
