About The Position
Wesley Living is seeking a self-motivated, empathetic Service Coordinator for its Graceland Gardens community in Memphis, TN. This faith-based organization, with over 50 years of experience managing affordable senior living communities, values Service, Compassion, and Integrity. The Service Coordinator will provide essential services to seniors, acting as a bridge between residents and resources to help them age-in-place with dignity. This role requires strong multi-tasking and time management skills.
Requirements
- Bachelor’s Degree or Higher in Human Services Field
- Driver’s License
Responsibilities
- Conduct initial and ongoing functional assessments to identify residents' needs.
- Help residents apply for programs like SNAP, Medicaid, and utility assistance.
- Establish and maintain relationships with local healthcare providers, home health agencies, and transportation services.
- Organize wellness clinics, financial literacy workshops, and social events to combat senior isolation.
- Provide immediate support and referral services during resident emergencies or transitions (e.g., hospital discharges or move-ins).
- Maintain accurate, confidential resident files in accordance with HUD (or relevant agency) guidelines and privacy laws.
- Assist in resolving conflicts between residents or between residents and management to maintain a peaceful living environment.
