About The Position
The Service Coordinator works directly with individuals enrolled in HCS, TxHml, and GR CFC programs in Tarrant County. The role helps individuals identify their needs, advocate for their preferences, and pursue personal goals and independence. The coordinator ensures services align with each person’s Person-Directed Plan (PDP), connects people with community resources, and monitors and coordinates essential support. The position focuses on advocacy, relationship-building, and consumer-driven services that support individuals and families.
Requirements
- Bachelor’s degree in applied behavior analysis, a behavioral services field, communication disorders, criminal justice, counseling, education, family studies, gerontology, educational psychology, health services, a human services field, human growth and development, medicine, nursing, physician assistant studies, psychology, rehabilitation, a social services field, social work, sociology, or special education.
Responsibilities
- Help individuals identify their needs and advocate for their preferences.
- Support individuals in pursuing personal goals and independence.
- Ensure coordinated services align with each person’s Person-Directed Plan (PDP).
- Connect individuals with community resources.
- Monitor and coordinate essential support.
- Provide consumer-driven services to individuals and families in Tarrant County.
