About The Position

The Resource Exchange (TRE) is seeking a Service Coordinator for their Early Intervention (EI) program. This role is responsible for understanding and facilitating the entire Early Intervention Process for eligible children from initial referral through transition. The coordinator will describe the Early Intervention Program to families, educate them on their rights under Part C of the Individuals with Disabilities Education Act, and ensure those rights are protected and all timelines are met. This position involves maintaining accurate child records in both state and agency databases, coordinating evaluations, collaborating with evaluation teams, and documenting results. The Service Coordinator will collect in-depth information from families to write meaningful plans that include child and family outcomes, and ensure families understand and support the service provision models used (Primary Provider and Coaching). They will collaborate with team members to meet family priorities and ensure the child's best outcome, keeping the Individualized Family Service Plan accurate and up-to-date. Building relationships with families, providing resource information, coordinating with community partners, and preparing families for transition out of the program are key aspects of this role. The position requires clear, effective, and professional communication, flexibility, and resiliency. Other duties may include participating in professional development opportunities, acting as a transdisciplinary team facilitator, translating program forms, and answering phone calls as needed. Driving to community locations is also a responsibility.

A Bachelor's degree from an accredited college or university; or a two-year degree plus two years of experience working with families and children with disabilities; or five years’ experience working with families and children with disabilities (experience must be specifically documented and verifiable on your application).
A working vehicle, valid CO driver’s license (unless active Military family), and auto insurance coverage with bodily injury and property damage liability coverage ($100,000/$300,000/$100,000 coverage recommended) are required.

Responsibilities

Understand and facilitate the entire Early Intervention Process for eligible children from initial referral through transition.
Describe the Early Intervention Program to families in an understandable and family-friendly way.
Educate families on their rights under Part C of the Individuals with Disabilities Education Act and ensure these rights are protected.
Ensure all timelines are met in accordance with state and federal regulations, including Prior written notice, informed parental consent, Native language, Access to records, Confidentiality, Dispute resolution, 45 day timeline, 28 day timeline, 6 month and annual review dates, Funding, and Transition timelines.
Maintain a complete and accurate child record for 55-65 children and ensure both the state and agency databases are accurate and up to date.
Coordinate evaluations, collaborate with evaluation team members, and document evaluation results.
Collect in-depth information about parent concerns, health history, and family activities to write a meaningful plan that includes routines-based child and family outcomes.
Ensure families understand the models of service provision (Primary Provider and Coaching models) and support team members in using these models.
Collaborate with team members to ensure family priorities are being met and the child has the best possible outcome.
Keep the Individualized Family Service Plan accurate and up to date.
Utilize family-centered practices to build relationships with families, contact them regularly, and provide information about resources as needed.
Coordinate and communicate with community partners to ensure families are accessing all available resources and building a network of support.
Prepare the family for transition out of the Early Intervention Program within the required timelines.
Utilize clear, effective, and professional communication at all times.
Demonstrate flexibility and resiliency in response to change.
Participate in professional opportunities (committees, workshops, conferences, journal readings) based on personal professional development plan.
Act as transdisciplinary team facilitator and support professional role release and role expansion.
Translate program forms for leads, as needed.
Occasionally answer a phone call that comes in from the front desk if no one else is available.
Drive to community locations as related to the job.
