About The Position

The Early Intervention Service Coordinator is responsible for effectively and efficiently managing a caseload and assisting each eligible family in identifying, locating, coordinating and monitoring mandated services for children birth to three years of age. This role involves initial screening for Early Intervention Services to determine family resources, priorities, and concerns, and to identify necessary supports and services. The Service Coordinator also tracks at-risk children who do not meet eligibility criteria but require tracking services. Key responsibilities include coordinating the Multi-Disciplinary Evaluation within 45 days of referral, facilitating and participating in the development, implementation, review, and evaluation of the Individual Family Service Plan (IFSP) and transition plan. The Service Coordinator follows up on health, safety, or other identified concerns and documents activities, billing MA if applicable. They assist families in accessing early intervention services and other services in the least restrictive environment, including arranging medical and health services and providing community resources. The role requires attending at least 24 hours of training annually, in addition to pre-service training relevant to early intervention, child development, community resources, or services for children with disabilities. Other duties may be delegated.

Requirements

Bachelor’s degree which includes 12 college credits in early intervention, early childhood, special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences.
One year of full-time or full-time equivalent experience working with or providing counseling to children, families or individuals with disabilities.
Must have Child Abuse, Criminal History, and FBI Clearances for employment.
Must have an automobile in good working condition with Pennsylvania driver’s license and adequate insurance coverage.

Responsibilities

Effectively and efficiently manage a caseload.
Assist eligible families in identifying, locating, coordinating, and monitoring mandated services for children birth to three years of age.
Complete initial screening for all referrals to determine resources, priorities, and concerns of the family, and to identify necessary supports and services.
Track at-risk children who do not meet eligibility for Early Intervention Services but meet criteria for tracking services.
Coordinate the Multi-Disciplinary Evaluation within 45 days of initial referral.
Facilitate and participate in the development, implementation, review, and evaluation of the Individual Family Service Plan (IFSP) and transition plan.
Follow up on any health, safety, or other concerns identified.
Document activities of monitoring in case comments and bill to MA, if applicable.
Assist families in gaining access to early intervention services and other services in the least restrictive environment.
Assist the family in arranging to receive medical and health services.
Provide the family with appropriate community resources.
Attend at least 24 hours of training annually, in addition to any pre-service training relevant to early intervention services, child development, community resources, or services for children with disabilities.
Perform other duties as delegated.
