About The Position

One Medical Seniors is seeking a full-time Nurse Practitioner or Physician Assistant to join its outpatient primary care practices in Shoreline, Washington. The role works closely with the medical director and contributes to both patient care and ongoing practice innovation. It offers the opportunity to deliver care in a value-based environment supported by a team, with a focus on high-quality care for older adults. One Medical Seniors uses a relationship-based care model for adults on Medicare and others who may need additional attention. Its practices feature smaller patient panels, team-based care, and technology designed to support care delivery. The role also includes weekly protected time during the workday for provider team building and discussion of challenging cases.

Requirements

At least 2 years of primary care experience.
Current licensure, or the ability to obtain licensure, in Washington state.
Ability to build successful relationships with team members and communicate effectively one-on-one and in groups.
Ability to thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes.
Board certification.

Responsibilities

Provide comprehensive primary care in an outpatient setting.
Partner with the care team and use coaching and integrated behavioral health models to engage geriatric patients in care and support behavior change.
Use One Medical’s electronic health record to plan care for individual patients and maximize impact across the patient population.
Work closely with the medical director to contribute to care delivery and ongoing practice innovation.
