About The Position

Rogue Community Health is looking for a new team member who is bilingual in Spanish and English, passionate about serving our community. The Roster Management Coordinator plays a vital role in supporting our mission of providing high-quality, patient-centered care, with a special focus on people who face challenges accessing services. Working closely with patients, care teams, and community partners, the Roster Management Coordinator helps ensure patients remain connected to the services they need to thrive. This role is responsible for growing, maintaining, and reconciling Rogue Community Health's Alternative Payment & Advanced Care Model (APCM) patient roster while proactively reaching out to patients who may be overdue for care or experiencing barriers to accessing services. The Roster Management Coordinator partners with teams across Analytics, Finance, Operations, and statewide healthcare organizations to ensure accurate patient enrollment, maintain data integrity, and support innovative healthcare delivery models that improve outcomes for our community.
